Minnesota Business Tax ID Number
All Minnesota Department of Agriculture (MDA) license application forms request information for workers’ compensation insurance information. A Minnesota Business (Tax) Identification Number must be provided or application forms received without this information, or improperly completed, may result in your application and payment being returned, or a delay in the issuance of your license.
Social Security Number
Provide Social Security Number or MN Business (Tax) Identification Number if available. Please note: A Social Security Number is not public data, therefore the MDA can not disclose this information without written consent from you unless specifically authorized by law to do so. Minnesota Statutes, Section 270.72 Subd. 4, gives the MDA legal authority to request the Social Security Number of license applicants.
Workers’ Compensation Insurance
Minnesota Statute 176.182 requires persons applying for a license to operate in Minnesota provide acceptable evidence of compliance with the Workers’ Compensation Insurance Law. If you are self-insured, please attach a copy of the exemption order from the Commissioner of Commerce authorizing self-insurance. On the license application form, you will need to provide the following information:
- Do you have paid or otherwise compensated employees in Minnesota?
- If yes,
- Name of Workers' Compensation Insurance Company
- Policy Number
- Effective Dates
Data Privacy
The data that you supply on a license application form will be used to process your application. If you do not have a Minnesota business ID you must provide us with your social security number (see Minn. Stat. section 270.72). We will not grant this license without it. Your social security number is private data under Minn. Stat. Section 13.49. No one will be given access to your private data except those permitted access by law, by your written consent, by a court order or by those department employees who require access in the performance of their official duties.
