To schedule appointments for testing sessions the applicators pesticide license number or item number must be entered to verify eligibility for testing services. Only applicators that have submitted an application or renewal for the current year with corresponding payment and required documents will be eligible to schedule exams.
New applicators will use the item number form their online payment confirmation receipt, as pictured below:
