If your complaint DOES NOT involve products containing the new dicamba formulation, i.e. Engenia, Tavium, or XtendiMax, or if you are unsure, DO NOT use this form. All unknown or other pesticide complaints should be submitted using the general Pesticide and Fertilizer Misuse Complaint Form.
